REQUIRED FORMS & DOCUMENTS
Completion and submission of the online General Release/Registration Form and the Code of Conduct are required for all student-athletes.
These must be completed before a student-athlete can:
Attend any of our sessions or practices, and CULTURE Tournament Team.
Document Submission Requirements
To ensure every athlete is verified and eligible to participate, parents or guardians must email digital copies of the following documents to cultureathletesaz@gmail.com
- Recent Headshot Photo – a clear image of the athlete’s face for identification. 
- Birth Certificate – used to confirm age and eligibility. 
- Secondary Photo ID – must include both a visible photo and date of birth (e.g., school ID, passport, or state-issued card). 
For our 7on7 CULTURE Athletes
In addition to the items above, 7on7 participants are required to submit a school-related document—such as a current student ID or enrollment verification—that clearly lists the athlete’s:
- Full Name 
- Photo 
- Current Grade Level 
All documentation must be received and approved before athletes can take part in any CULTURE Tucson Athletics practices, workouts, or competitions.
